Release of Liability
In exchange for participation in the activity organized by MK Glass Designs, Melissa Kluczynski and or their agents for use of the property facilities and services of MK Glass Designs and Melissa Kluczynski, I agree to adhere to the direction and guidelines described below and follow all instructions given to me at all times.
You will have the opportunity to create an art piece using some or all of the following equipment and materials: stained glass sheets and pieces, glass nippers, glue, glass cutters, running pliers, add ins, safety glasses, gloves, scissors, grout, foil, solder, soldering iron, soldering iron stand and other tools as necessary.
I recognize that there are certain inherent risk involved with participation of the stained glass activity and further release and discharge MK Glass Designs, Melissa Kluczynski and or their agents…

Refund Policy - Refunds will be issued with a written request submitted at least 7 days prior to the start of the activity. Refund requests received within 7 days of the activity will receive a credit for a future class which is valid for one year. Refund requests received after the start of the activity will not be honored, unless due to a documented illness or injury. Unless an activity is cancelled by MK Glass Designs, all refunds and credits will incur a transaction fee of 10%.
