Please note this event is held in the barn. When arriving at the park go all the way to your right, you will see a white house and a red barn. We will be in the red barn.

By purchasing a ticket to this event I agree to the following:

Release of Liability

In exchange for participation in the activity organized by MK Glass Designs, Melissa Kluczynski and or their agents for use of the property facilities and services of MK Glass Designs and Melissa Kluczynski,  I agree to adhere to the direction and guidelines described below and follow all instructions given to me at all times.

You will have the opportunity to create an art piece using some or all of the following equipment and materials: stained glass sheets and pieces, glass nippers, glue, glass cutters, running pliers, add ins, safety glasses, gloves, scissors, grout, foil, solder, soldering iron, soldering iron stand and other tools as necessary.

1. I recognize that there are certain inherent risk involved with participation of the stained glass activity and further release and discharge MK Glass Designs, Melissa Kluczynski and or their agents for injury, loss, or damage arising from the use of my presence upon the facilitates whether caused by myself, MK Glass Designs, Melissa Kluczynski and or their agents.
2. I agree that I will observe and obey all posted rules and verbal instructions warnings and further agreed to follow any instructions given by the staff, representatives of MK Glass Designs, Melissa Kluczynski and or their agents.
3. I agree and indemnify MK Glass Designs, Melissa Kluczynski and or their agents their staff, agents, designers and representatives against all claims causes of action, damages, judgments, costs or expenses, including attorney fees or other litigation costs which may arise from my use our presence upon the facilities and instructions of MK Glass Designs, Melissa Kluczynski or their representative..
4. I agree to pay for any and all damages to the facilities, tools, supplies and retail items by my negligent, reckless or willing actions.
5. Any legal or equitable claim that may arise from participation in the above shall be resolved under Maryland law.

I have read and understand this release of liability. I understand by puraching a ticket to this event, I voluntarily surrender certain legal rights.
